[Katrina-IT-Volunteers] RE: Summary information sheets, folders

All-

I worked at the computers the first 3 days and have been working on a
summary sheet for evacuees (and volunteers) to use and complete, based
on all the information everyone has been posting.  Glenn mentioned
another volunteer is also working on this - if it's YOU, please connect
with me so we can work together ;)

I've attached a rough draft in PDF (small enough to attach).  Please
send suggestions on changes and additions.  I'll be heading to the
convention center on Monday and want to have copies ready.

Thinking these can be on a bright color paper to catch attention.  If we
use these, we could consolidate information people have on slips of
paper into one reference sheet.
Major topics included in the sheet include:
- Contact information on evacuee (email address, PO Box, etc)
- FEMA application information (ID#, user, password + changed password,
PIN + changed PIN)
- Contact Numbers (disaster relief, Food stamps, WIC, social security
checks, etc)
- People Locator information (sites to search, if found information on
where found - site, location, contact info, etc.)

Does anyone know if folders have been offered to those at the shelter?
Might be a nice thing if we could give people a folder to keep all their
paperwork in.  I might be able to get this donated from my work - have a
request in now.
